General Summary:      Responsible for planning, developing, organizing, implementing, evaluating, and directing social service programs of the skilled nursing facility.

Position Responsibilities:

  • Responsible for providing services to meet the social and/or emotional needs that affect our residents’ ability to achieve their highest level of function.
  • Participate in the development of the resident’s individualized comprehensive plan of care.
  • Participate in QAPI and interdisciplinary meetings.
  • Maintains regular social service progress notes including initial and quarterly assessments.
  • Responsible for all aspects of discharge planning including coordinating discharge meetings, scheduling transportation, coordination of DME, and completing appropriate discharge paperwork.
  • Serves as the Grievance Officer and Abuse Coordinator for the facility.
  • Educates staff regarding resident psychosocial issues.
  • Responsible for accurate and timely completion of PASRR
  • Responsible for completion of Medicaid applications as needed.
  • Provides crisis intervention, emotional support and counseling as needed to residents, families and staff.
  • Assists with residents and family members with Advanced Directives that accurately reflect their wishes.
  • Perform other duties that may be assigned by the Administrator.


  • EDUCATION REQUIREMENTS:
  •  
  • Bachelor’s degree or master’s degree in social work

  • REQUIRED EXPERIENCE:

  • At least two years recent experience in a health care setting
  • Geriatric experience preferred
  • Must possess excellent communication and organization skills
  • Relates positively to residents, families and staff members
  • Basic computer knowledge and ability to learn computer software
